Patients will be able to connect with:
Citizens Advice Bureau – Community Advice Workers for support with money and finance-related concerns
Workwell for employment advice and help returning to work
Social Prescribers for health and wellbeing support, including warmth on prescription and local signposting
PCN Care Coordinators for NHS App support, Patient Participation Group (PPG) activities, and guidance on health-related queries
CAC (Carers and Communities) for carer advice and support
Healthcare professionals, including Healthcare Assistants, Nurse Associates, Clinical Pharmacists, and Pharmacy Technicians, for health advice and interventions
In addition to advice and support, drop-in sessions will also offer a range of health services, including:
Blood pressure (hypertension) checks
Atrial fibrillation (AF) monitoring
Cancer screening information and advice
