About NIGHTINGALES PCN.

We’re a group of six GP practices, supporting patients in Redditch, Bromsgrove, Solihull, South Birmingham and Warwickshire. We work together with our local primary care providers and community organisations to make it easier for our patients to access the healthcare they need.

Our Values/Vision

  • As a network of 6 GP practices, we will work together for the health and well-being of the communities we serve.

  • We will value the staff and structures within each member GP practice and find ways to effectively involve and inform staff of the intentions and developments across the PCN.

  • We will work collaboratively wherever possible – optimising workforce skills and harmonising systems and processes where agreed, in a reciprocal way in order to benefit all member practices.  We will share workforce and resources where it makes sense to do so.

  • We value the patient voice and will seek to involve patients via PPGs in order to secure the necessary level of patient input to effectively inform and optimise service developments and resource available. 

  • We value education and career development and will share training and development opportunities including processes such as appraisal, training and supervision.

  • We will work collaboratively via a single PCN voice (working with neighbouring PCNs where it makes sense to do so) in order to influence the wider system and work effectively and in an integrated way across health, social care and the voluntary sector.

What is a Primary Care Network?

 

Since the NHS was created 70 years ago, the population has grown - and we are living longer. Many, however, are living with long-term physical and mental health conditions that mean they need to access their local health services more often.

To meet the needs of our changing population, GP practices work together with community, mental health, social care, pharmacy, hospital and voluntary services in their local areas in groups of practices known as Primary Care Networks - or PCNs.

PCNs build on existing services, allowing for more proactive, personalised, better coordinated and more integrated health and social care for people closer to home.
